Corrections log
A permanent, public record of every clinical error found in Open Sim Lab and what was done about it.
- Entries are appended. Nothing here is ever deleted or rewritten, including entries that are embarrassing.
- Each entry states what was wrong, the potential educational impact, who reported it, what changed, and which build carried the fix.
- An error that could teach an unsafe practice is triaged as urgent, and the affected content is disabled in the next build regardless of the release schedule.

No corrections have been recorded yet, because no clinical review has yet taken place. This log is not empty because the content is right; it is empty because nobody qualified has looked at it. The review-status page lists every item and the label it is published under, and the governance page names the empty board.
How to report something
Every playable scenario carries a Help us improve this control. It sends only the scenario, its version, the public practice context, a category, and an optional 160-character note, after showing you a preview of exactly that. Do not include real patient or learner information; there is no field that wants it.
Where the separately deployed report service is unavailable, the control says so rather than pretending to queue anything, and invited reviewers can still record notes at the content-review page and return the exported file through their invitation channel.
What happens next
- Acknowledged
- A usable report is acknowledged within 5 working days. Where public intake is unavailable — including on a static-only fork — the control says so rather than silently sending anywhere.
- Reproduced and source-checked
- A report is detection, not review. No item's status changes on report volume, only after the problem is reproduced and checked against an authoritative source.
- Urgent, if it could teach unsafe practice
- The affected content is disabled in the next build regardless of the release schedule, and this log records the timeline.
- Recorded
- The correction is appended here permanently, with what was wrong, what a learner might have concluded, what changed, and which build carried the fix.
